Required Documents for Upload: You’ll need to upload the following documents when completing the application:
Birth certificate
Immunization form
Most recent report card
Test results
Discipline report
Bring Originals to Your Tour: In addition to uploading documents, please bring the original copies of the following to your tour:
Birth certificate
Immunization form
Physical form (with parent signature, not uploaded)
Report cards
Test results
Discipline report
Once you submit your application, you won’t be able to make changes or upload additional documents.
Current Teacher/Principal Recommendation Form:
The recommendation form must be emailed to info@dcaeagles.com or faxed to 352-489-5760.
If applicable, please email any 504 plans or IEPs.
Next Steps:
You must submit all forms and the $200 registration fee to schedule an interview after your tour.
In most cases, a member of our administration will conduct the interview immediately following your tour.
